What Medicare Costs in 2026
The numbers that matter are the ones you pay every month and the ones that can appear when you actually need care.
2026 Medicare costs at a glance
Premium is only the first number
A $0-premium Medicare Advantage plan does not mean $0 health-care costs. You still generally pay your Part B premium and may owe deductibles, copays or coinsurance as you use services. With Medigap, the monthly premium is usually higher, but specified Medicare cost-sharing may be reduced substantially depending on the plan letter.

Part A costs are based on benefit periods
For 2026, the Part A inpatient hospital deductible is $1,736 per benefit period. That wording matters: the hospital deductible is not described as one simple annual deductible. Longer stays can also involve daily coinsurance.
Higher income can increase Part B and Part D costs
Medicare uses income-related monthly adjustment amounts, commonly called IRMAA, for some higher-income beneficiaries. In 2026, the first IRMAA tier begins above $109,000 of modified adjusted gross income for an individual return and above $218,000 for a joint return.
